Consent with Personal Data Processing

The present consent is granted pursuant to the Regulation (EU) of the European Parliament and of the Council No. 2016/679 of 27 April 2016 on the protection of natural persons with regard to the processing of Personal Data and the free movement of such data (hereinafter referred to as “GDPR”), pursuant to act No. 101/2000 Sb. on Personal Data protection (hereinafter referred to as “ZOOÚ”) and pursuant to act No. 480/2004 Sb. on certain information society services and on the amendments to certain other acts.

I hereby grant my consent, to Continental Gallery s. r. o, with its registered seat at Lhotská 1011/9, Praha 9 – Horní Počernice, CP 19300, Czech Republic, ID No. 26 19 43 50, registered in the Commercial Register maintained by the Municipal Court in Prague, Section C, File 78649 (hereinafter referred to as “Controller”) as a Personal Data Controller to process my Personal Data, until the withdrawal, for the purposes of:
 
 a) performance of the purchase Agreement or in connection with the provided license for the e-book or to offer and provide suitable commercial offers related to the concluded purchase Agreement or in connection with the provided license, both in electronic and hard copy;

 b) in connection with the provided license to use the COGYBOOKS application or with offering and providing suitable commercial offers related to the provided license to use the COGYBOOKS application, both in electronic and hard copy.

 
I hereby grant my free, specific, unequivocal and informed consent to process, store, use and evaluate the following Personal Data:
 
The Personal Data that will be processed based on the consent include:
 

  • name and surnames(s)  
  • country
  • delivery address
  • invoice address
  • e-mail
  • phone number


My consent is voluntary, and I am conscious of the fact I may withdraw my consent at any time in the same manner it was granted. Other information related to the processing of Personal Data is contained in the Terms and Conditions of the Controller.

At the same time, I understand that not granting or withdrawing my consent may result in the impossibility to offer or provide some performances related to:
 

  1.    a) Purchase Agreement or License provided for a e-book;
  2.    b) license provided for the use of the COGYBOOKS application
     

 However, providing the performance is by no means conditioned by granting this consent.
